INVASIVE PROCEDURES REGISTRATION: How to Apply
Please complete the application forms below
and submit these to the Licensing Section at
Melton Borough Council with your payment. There is
no annual renewal fee.
There are separate application forms for:
- Registering individuals for acupuncture, cosmetic piercing,
tattooing or electrolysis
- Amending the names on an existing Registration
- Registering a premise for acupuncture, cosmetic piercing,
tattooing or electrolysis
If you are setting up for the first time you
will need to apply to register the premises
and personnel.
If you are changing the personnel on an existing registration
you will need to submit an
amendment application plus £50.
Once an application has been made and the fee
has been paid,
an Environmental Health Officer will undertake
an inspection of the premises. If the officer considers
the applicant's procedures and the premises to be suitable, then
registration will be approved. The officer will also
check compliance with bye-law requirements as well as ensuring that
general health and safety requirements are being met.
The officer will be pleased to offer any advice or give any
help if you are unsure as to how to meet these obligations.
