Massage and Special Treatment Licence

To register with Melton Borough Council for massage and special treatments please complete and return this application form.  There is no fee for registration, unless you are also undertaking any invasive procedures such as electrolysis or ear piercing, for which a separate form should be completed.  Please see the separate section on this website for accupuncture.

Completed forms should be sent to Environmental Health.
